The Eligibility Determination Process

A CDDO Access Specialist will review the information gathered from the Application Packet.
All authorization forms will be sent to the applicable entities to gather all relevant diagnostic records & data.
After receiving all of the required data, a review of the information will be completed to make a final determination.
You will then be notified via written letter of your “approval” or “denial” of the Eligibility Determination Phase by the CDDO.
If you are determined eligible, and the individual is over the age of 5, a secondary appointment will then be scheduled for a functional assessment to enter into the State of Kansas system. The individual applying for services must be present for this screening.  Also if determined eligible the CDDO will offer options counseling and a choice of a Targeted Case Manager.  
According to the Developmental Disabilities Reform Act (DDRA), eligibility criteria for having a developmental disability is defined as:

*** Intellectual Disability ***

Substantial limitations in present functioning that is manifested during the period from birth to age 18 years and is characterized by significantly sub-average intellectual functioning existing concurrently with deficits in adaptive behavior including related limitations in two or more of the following applicable adaptive skill areas:
Communication, Self-care, Home living, Social skills, Community use, Self-direction, Health and safety, Functional academics, Leisure and work
Or:

*** Developmental Disability ***

A Severe/Chronic Developmental Disability, which:
Is attributable to a mental or physical impairment, or multiple sensory impairments, a combination of mental and physical impairments, physical and sensory impairments, mental and sensory impairments or a condition which has received a dual diagnosis of intellectual disability and mental illness;
Is manifest before 22 years of age;
Is likely to continue indefinitely;
Results, in the case of a person five years of age or older, in a substantial limitation in three or more of the following areas of major life functioning:
Self-care, Receptive and expressive language development and use, Learning and adapting, Mobility, Self-direction, Capacity for independent living, Economic self-sufficiency;
Reflects a need for a combination and sequence of special interdisciplinary or generic care, treatment, specialized communications techniques or other services which are lifelong, or extended in duration and are individually planned and coordinated; and
Does not include individuals who are solely and severely emotionally disturbed or seriously or persistently mentally ill or have disabilities solely as a result of the infirmities of aging.

***  Children with a Developmental Disability  ***

A child with a developmental disability applies to children under the age of six and includes the following criteria: Is attributable to a mental or physical impairment or a combination of mental and physical impairments; and Is likely to continue indefinitely; and Results in at least three (3) developmental delays as measured by qualified professionals using appropriate diagnostic instruments or procedures and
Reflects a need for a combination and sequence of special, interdisciplinary or generic care, treatment, or other services which are lifelong or extended in duration and are individually planned and coordinated; and
Does not include individuals who are solely severely emotionally disturbed or seriously and persistently mentally ill.
